'No water' notification form

This form is to register with the Christchurch City Council that water is not currently being supplied to your house or business. Before completing this form please confirm that the street valve ('toby box') to your property water is on. This is generally located in the footpath or grass beside your front fence. The tap/valve is normally inside either a buried rectangular black plastic box or a circular metal chamber. The tap/valve is 'on' when turned anticlockwise.
